• Application for 2-100 and 3-100- Updated fees.
• Water Obstruction Determination Form- To determine if a dam can be reduced to a water obstruction.
• Limited Class A Dam Inspection Form- A safety inspection form for class A (low) hazard dam.
• Inundation Zone Notice Form- Any owner of a dam who sells, subdivides, or otherwise transfers any land within the inundation zone must inform the Register of Deeds office. Failure to submit this to the Register of Deeds may result in paying part of future inspection fees.

Dam Safety Inspection Frequency Adjustment Diagrams- These diagrams outline the scenarios of a hazard class upgrade at no-fault of the owner. The inspection frequency increases as a result of the hazard class upgrade. The inspection fee is prorated to the new inspection frequency, but the total fee remains the same over a normalized time period equivalent to the previous, lower hazard classification.
